Prescription refill requests should be made by calling the office, during normal business hours. When calling please have the following patient information available: name, date of birth, prescribing physician, prescription name, prescription dosage amount and frequency, and the name and number of your preferred pharmacy. Requests after office hours will only be handled in emergency situations. Please contact us a few days prior to running out of medication.
